    Cannabis Topicals: Creams, Patches and What Actually Absorbs

    Topical and transdermal are not synonyms, and the difference is the whole point.

    Topicals are how many people first try cannabis, because the appeal is obvious: targeted relief, no intoxication, nothing to inhale. The category is also full of products containing so little cannabinoid that no mechanism could produce an effect.

    Two ideas let you sort through it.

    Topical Versus Transdermal

    These get used interchangeably in marketing and mean different things.

    Topical products are designed to act locally. Cannabinoids interact with receptors in skin and nearby tissue without meaningful systemic absorption. Creams, balms, salves and lotions are topical. They do not intoxicate, and they will not normally produce a positive drug test.

    Transdermal products are designed to cross the skin into the bloodstream. This requires permeation enhancers, because cannabinoids are lipophilic and the outer skin layer resists absorption. Properly formulated transdermal patches achieve systemic delivery, which means a THC patch can intoxicate and can cause a positive test.

    If a product says transdermal but is a simple cream with no permeation strategy, the label is aspirational.

    Why Absorption Is the Central Problem

    The stratum corneum, the outermost skin layer, is an effective barrier evolved to keep things out. Cannabinoids are highly lipophilic and poorly water soluble, a combination that makes them slow to cross.

    That is why formulation matters more than cannabinoid content alone. Approaches used to improve penetration include liposomal and nanoemulsion delivery, oil bases with penetration enhancers such as certain alcohols and terpenes, and patch systems with dedicated enhancer chemistry.

    It also explains why deep structures are unrealistic targets. A cream on the skin over a hip joint has to cross skin, then fat, then reach a joint capsule. Fingers, wrists, elbows, knees and superficial muscle are plausible. Hips and lower back are not, whatever the label claims.

    What the Skin Receptors Suggest

    Skin contains CB1 and CB2 receptors, and they are present on keratinocytes, sebaceous gland cells, immune cells and sensory nerve endings. The endocannabinoid system participates in skin barrier function, sebum production, inflammation and itch signalling.

    That gives topicals a plausible basis for localised pain, itch and inflammatory skin complaints. Human trial evidence remains limited, which is the recurring theme.

    Reading a Topical Product Honestly

  1. Look for total milligrams, not percentages. "Contains CBD" is meaningless. A 100 mL jar with 50 mg of CBD spread across it delivers a trivial amount per application.
  2. Compare against oral doses. Research doses of CBD run into hundreds of milligrams. A jar containing 100 mg total is not in the same universe.
  3. Check for a certificate of analysis. Topicals are often less regulated than ingestibles, and contamination applies to them too.
  4. Notice the other active ingredients. Menthol, camphor, capsaicin and arnica have their own well-known sensory effects. If a balm feels effective, some of that is likely the menthol.
  5. Be sceptical of bath products. A CBD bath bomb disperses a small amount of lipophilic compound into a large volume of water. The mechanism for meaningful absorption is not there.
  6. The Wellness Product Problem

    Cannabinoids now appear in lotions, serums, mascara, sparkling water, pillows and pet treats. Most of this is brand positioning rather than pharmacology, and the pattern is consistent: cannabinoid content far below any studied dose, no certificate of analysis, and claims that would require systemic delivery from a product designed not to achieve it.

    The useful filter is to ask what dose the product delivers, by what route, and compared with what studied amount. Products that cannot answer that are cosmetics with a marketing ingredient.

    Where Topicals Genuinely Make Sense

  9. Itch and irritated skin, where the receptor rationale is reasonable.

  11. The risk profile is genuinely low, which is the strongest argument for trying them. Skin irritation is the main adverse effect, and patch testing a small area first is sensible.

    Bottom Line

    Topical means local action without intoxication, transdermal means systemic delivery with both. Absorption through skin is the limiting factor, which makes formulation and total milligrams the things worth checking, and deep joints an unrealistic target.
