Cannabis gets treated as a supplement, which leads people to assume it sits harmlessly alongside their prescriptions. It does not. Cannabinoids are metabolised by the same liver enzymes that clear most pharmaceuticals, and CBD in particular inhibits several of them.
That means CBD can raise blood levels of other drugs without anyone changing a dose.
The Enzymes Involved
CYP3A4 alone handles a very large share of prescription medicines. That breadth is why the interaction list is long rather than exotic.
A useful shorthand: if your medication carries a grapefruit warning, treat CBD with the same caution. Grapefruit inhibits CYP3A4 through a different mechanism, and the drugs flagged for it overlap heavily with those CBD affects.
The Interactions With Serious Consequences
Warfarin. CBD can raise warfarin levels by inhibiting CYP2C9, increasing INR and bleeding risk. Case reports describe exactly this. Anyone on warfarin who starts CBD needs INR monitoring and probably dose adjustment. This is the interaction most likely to cause immediate harm.
Clobazam. CBD raises levels of N-desmethylclobazam, clobazam's active metabolite. This drove much of the sedation observed in the Epidiolex epilepsy trials, and clobazam doses commonly need reducing when CBD is added.
Valproate. Combining valproate with CBD is associated with elevated liver transaminases, which is why liver enzyme monitoring is part of the Epidiolex protocol rather than optional.
Sedatives and CNS depressants. Benzodiazepines, opioids, sleep medication, sedating antihistamines and alcohol all combine additively with cannabis. This is not a metabolic interaction, it is straightforward pharmacodynamic stacking, and it is the most common real-world problem because people do not think of alcohol as a drug interaction.
Immunosuppressants. Tacrolimus and cyclosporine have narrow therapeutic windows and are CYP3A4 substrates. Unintentionally raising their levels is dangerous, and this matters especially for transplant recipients.
The One People Never Expect
Smoking cannabis induces CYP1A2. This has nothing to do with cannabinoids. Combustion produces polycyclic aromatic hydrocarbons, and those induce CYP1A2 exactly as tobacco smoke does. Inducing an enzyme speeds clearance, so drug levels fall.
Clinically this matters most for clozapine, olanzapine and theophylline. A patient who smokes cannabis may need a higher dose, and someone who stops smoking can see levels rise into toxicity without changing anything.
Note the implication: this effect comes from smoke, not from THC. Switching from smoking to edibles removes it, which can itself destabilise a dose.
Interactions Worth Knowing About
Why Dose Determines How Much This Matters
Enzyme inhibition is dose dependent. A 10 mg CBD gummy is unlikely to move warfarin levels. The several hundred milligram daily doses used in epilepsy treatment clearly can. Most documented interactions come from higher therapeutic dosing.
That is a reason for proportionate concern rather than dismissal. The problem is that people take escalating CBD doses over time without treating it as a dose increase in a drug.
What To Actually Do
1. Tell your prescriber, including about CBD. Framing it as a supplement is the mistake. 2. Check for a grapefruit warning on anything you take, as a rough first filter. 3. Start low and change one thing at a time, so an effect can be attributed. 4. Get monitoring where it exists. INR for warfarin, liver enzymes with valproate, drug levels for narrow-window medicines. 5. Do not stop prescribed medication to make room for cannabis.
Bottom Line
CBD inhibits CYP3A4, CYP2C19, CYP2C9 and CYP2D6, so it can raise levels of a wide range of prescriptions. Warfarin, clobazam and immunosuppressants are the interactions with the sharpest consequences. Sedative stacking is the most common. And smoking cannabis induces CYP1A2, which lowers clozapine and olanzapine levels through the smoke rather than the cannabinoids.
